While 86% of facilities have a medication reconciliation process in place, most of these are paper-based systems and few pharmacy directors are thrilled with their existing approach. The biggest roadblocks to implementing automated systems are a lack of budget coupled with an insufficient awareness of the solutions available in the market. Despite these drawbacks, most of those without an automated system are planning to implement one.
