Detecting and Responding to Drug Diversion
By Kimberly New, BSN, RN, JD

Detecting institutional drug diversion is a challenging task that requires creativity and collaboration between multiple departments, particularly pharmacy and nursing. Diverters are invested in concealing their activity and aware that the institution is attempting to discover and thwart their actions. Their focus is on outwitting surveillance in order to feed their addiction or perpetuate their enterprise. To stay ahead of these activities, detection should rely on the aggressive use of analytics, including pattern recognition in the transaction history beyond the detail of statistical outlier reports. Assessing the Efficiency of CSTDs for Compounding
By Fouzia Berdi, PharmD; Michael F. Powell, MS, FASHP; Christine Sanz, RPh; Richard Gonzalez, PharmD, MBA; and Fred Massoomi, PharmD, FASHP

Ensuring the safe handling of hazardous drugs (HDs) must be a primary concern for every health care worker who comes into contact with these medications. Given the potential dangers introduced by improper management and handling, health care organizations are wise to invest the necessary time and resources in a comprehensive HD safety program. In reviewing the prevailing guidelines, the number of steps required to safely compound HDs may appear daunting. However, safe handling of HDs is critical to their effective use in treating disease. Therefore, systems used to ensure the safety of critical tasks, including closed-system drug-transfer devices (CSTDs), should be carefully considered. Contrary to common belief, when staff is properly trained and are experienced CSTD users, the time required to compound CSPs using CSTDs does not differ significantly from the time it takes to compounding using a needle and syringe. To read about how the Nebraska Methodist Hospital pharmacy department conducted a time-motion study comparing the time required to compound an IV piggyback dose from a liquid drug vial using three CSTDs, CLICK HERE.
