Who we are?
Frontizo established in 2017, is a group of clinicians, management consultants, and developers, who came together to provide VBC services to various settings. Frontizo spread its wings in the outpatient setting, working with a multitude of practice sizes and specialties. Frontizo has worked with some of the highest performing ACOs and MA plans in multiple states across the country. Frontizo’s current focus is delivering technology and VBC services in the PAC setting to drive down readmission rates. Frontizo currently works with SNFs, ALFs, CCRCs, and physiatry groups across the midwest.
