OZtech’s Medication Reconciliation System is a Web-based, fully integrated system that helps hospitals avoid medication errors and comply with TJC’s requirements for patient safety. The system ensures that a complete and accurate list of a patient’s medications is obtained upon admission, updated during the course of care, and communicated to the next provider of care. Upon discharge or transfer, the system generates all the required reports, prescriptions, instructions to patient, taper calendars, and can generate custom reports so administrators can track and measure compliance and report it to regulatory agencies as needed.
