OZtech's medication reconciliation system is a Web-based, fully integrated system helping hospitals avoid medication errors and comply with TJC requirements for patient safety. Embedded in the system are SureScripts-RxHub’s e-prescribing network and patient pre-admit medication histories. The system ensures that a complete and accurate list of patient medications be obtained upon admission, updated during the course of care, and communicated to the next provider of care. Users can com- pare side-by-side pre-admit and current medication lists, and maintain the lists across the continuum of care. Upon discharge or transfer, the system generates all required reports, prescriptions, instructions to patient, and taper calendars. The system can generate custom reports to enable administrators to track and measure compliance as needed.
