Given that fewer than 10% of US hospital pharmacy directors are confident that they currently meet all of the requirements of USP <800>, significant effort needs to be brought to bear in hospital pharmacies nationwide in order to meet the looming December 1, 2019 deadline.

In addition to the physical improvements many facilities must undertake to achieve compliance, there are a variety of policy and procedure updates that pharmacy should review, including establishing a medical surveillance plan, conducting regular HD spill simulations, and implementing HD lot number tracking.
