A Checklist for Medication Safety

The process of improving medication safety across the spectrum of patient care is manifold, and can be an intimidating road for hospitals to embark upon. However, identifying the areas that present the most risk to patients is an important first step.

The following medication safety checklist, courtesy of the California Institute for Health Systems Performance (CIHSP), contains a list of processes and procedures that are known to decrease medication error rates. CIHSP recommends that hospitals use the list to reveal practice areas that require improvement. An evaluation of your completed checklist will direct your attention to particular practices that demand a more comprehensive review.

On its website, www.cihsp.com, CIHSP has made available many downloadable resources, including this checklist, for hospitals seeking to improve their policies and procedures.
