As the ranks of TJC surveyors now include more pharmacists, notably, those facilities inspected by a pharmacist are more likely to be cited or receive a recommendation related to compounding practices. Further underscoring the need for vigilance and compliance in compounding practices, patient incidents involving compounding errors rose to 34% this year.
