Recognizing the inherent danger in preparing medications on the floor, fewer facilities are permitting this practice outside of emergent situations. Too often, however, nurses are not prepared for these emergencies. In 47% of hospitals, pharmacists are not providing compounding training to nurses. Despite the fact that practical testing is an ideal way to ensure that the learner has absorbed a didactic lesson, very few facilities require annual media fills for their nursing staff. Underscoring the need for more in-depth training for nurses, more than half of nurses’ media fills have shown contamination.
